Independence and Life Skills for Transition Age Youth and Young Adults
We believe that young people are the experts in their own lives and we are excited to learn from each and every one of them and support them with their future goals.
Creative Collaborations is committed to supporting youth and young adults between the ages of 16-26 as they transition into independent living situations through mentoring and case management. We support young people in achieving their goals as they forge their own paths as successful and driven young adults. Support includes: access to basic needs, applications for public assistance, completing high school and entering higher education/trade programs, career exploration, transportation support, mental healthcare, and strengthening life skills.

- Always youth centered. We believe that youth are the experts in their own lives and we are committed to stepping aside and letting them lead us in the collaboration process.
- Staying up to date with current issues and trends. We are committed to ensuring we are well informed and well educated on the issues young people are facing in our communities and how we can mitigate those issues as best we can.
- Trauma informed. Creative Collaborations is committed to being trauma informed in all things we do and ensuring we are always adding to the healing rather than harming our clients.
- Affirming. Creative Collaborations is responsible for ensuring we are learning and doing the work to affirm all identities and creating a space that is safe for all people.
What to Expect:
- Intake Process - each young person will meet with a member of the Creative Collaborations team to discuss their current needs, what they hope to learn, any current barriers that are currently standing in the way of their success, and potential goals they may want to work on.
- Collaborator Assignment - The Creative Collaborations team will meet together to discuss each young person and determine which Life Skill Collaborator best fits their current needs and goals.
- Agreed Upon Plan - The Life Skills Collaborator will meet with the young adult to clarify and solidify the goals the youth has chosen to focus on. Together, they will determine a preferred mode and frequency of communication and develop a timeline that fits their individual needs and plan. The Life Skills Collaborator will partner with the young person to measure their progress and adjust the goals as needed.
